EHHOP Diabetes Screening Guidelines
Population to screen:
- Everyone >45 years of age
- Adults with HTN (blood pressure>140/90 mmHg or on therapy for hypertension)
- Adults with HLD
- Overweight (BMI ≥25 kg/m2) or Age >30 + >1 risk factor
- Physical inactivity(defined as <30 minutes activity such as walking every day)
- First-degree relative with diabetes
- High-risk race/ethnicity (eg, African American, Latino, Native American, Asian American, Pacific Islander)
- A1C ≥5.5% or metabolic syndrome on previous testing [3]
- Delivered a baby weighing>9 pounds or history of gestational diabetes mellitus
- Polycystic ovary syndrome (PCOS)
- Clinical conditions associated with insulin resistance (eg. severe obesity, acanthosis nigricans)
- History of CVD
- Smoking testing
Screening Test: Hemoglobin A1C
Frequency of screening:
- Every 3 years
- for patients >45 years without risk factors.
- For patients <45 years with risk profiles detailed above
- Annually
- For pre-diabetics (Hgb A1C >5.7)
- If risk profile changes dramatically indicating clinical need for retesting
