This referral requires EMC. Please submit the ancillary clinic referral form or email Chief EHHOPhtho TS (helen.liu@icahn.mssm.edu) for a NYEE referral.

Senior Clinician

Place an outside referral for the referral in EPIC and make sure attending signs the order. Directions below on how to submit a general Ophtho referral for St. Lukes/MSM in EPIC

Order: Consult to Ophthalmology (9024)

Select “Hospital Based Practices Referral” Site: New York Eye and Ear Input reason for consult in “comments” section Referring provider contact number: EHHOP main number Referral type: most often will be "consult, diagnostic procedure, treat".

Add the referral to the ACT Referrals Spreadsheet, "Referrals Request" tab and include the patient's availability for scheduling (day/time).

Before clinic ends, communicate the necessity and urgency of the referral to the TS.

Teaching Senior

Verify there is an active order in Epic.If not already done by the SC add the referral to the ACT Referrals Spreadsheet, "Referrals Request" tab and include the patient's availability for scheduling (day/time).

Please note that NYEE does accept walk-ins which do not require an appointment to be scheduled, but that we should still send medical records and a referral letter and counsel the patient on applying for the sliding scale.

Access to Care Team

Confirm patient has active charity care (CC). If patient does not have CC, fill out REAP referral form to obtain Emergency medicaid (EMC/CC)Obtain patient availability for appointment and note in their ACT Problem List in Epic.Give the patient a metro card if patient needs one for this referral.EHHOPhtho clinic managers will make the appointment with NYEE.The ACT Case Manager should call the patient to see if he/she can attend the appointment.The EHHOphtho TS will fax over the patient's past medical records along with a referral letter.NYEE will fax medical records to Dr. Meah's fax number (provided on the referral letter)

Cost

The cost of an appointment for patients with NYEE sliding scale coverage is $10. Surgeries will cost approximately $100 per procedure.

Wait time

It takes about 3 months for an ophthalmology appointment to be scheduled.

Appointment Hours: 8:00 AM - 3:30PM, Monday - Friday
Walk-In hours are any time between 8:00AM and 9:30PM, but patients have a better chance of being seen if they walk-in around 5pm. It can take up to 3 hours for a walk-in patient to be seen.
