Transitioning Patients Out of EHHOP

Patients are transitioned out of EHHOP when they either acquire insurance or if they are a new patient that does not fall in our catchment area (CM's handle this via screening over the phone). Patients that already are receiving care at EHHOP but leave our catchment area are permitted to stay at EHHOP.

Email Chief TS and ACT chairs to bring this to their attentionMake sure patient is medically stable, clearance will be given by Chief TSAfter receiving medical clearance have a preliminary conversation about transitioning with the patient use the Transition ScriptHelp the patient select a new Primary Care Physician (for insurance acquisition) and Pharmacy using the resources provided (EHHapp, spreadsheet, etc.)Set up an appointment for the patient at their new care facilitySet up a final appointment for the patient at EHHOP where there medical records will be given to them and their care will be officially transferred. Until this point it should be made clear to the patient that they can come to EHHOP with their care.Include ACT Chairs and Chief TS in all communications throughout the transition processEmail Chief CM once the transition is complete to make sure any further EHHOP appointments are canceled for this patientRemove Dr. Meah as the patient's PCP by clicking on her name in the first page of patients chart in EPIC, also change the pharmacy by using the demographics tab.

How to help a patient select a new Primary Care Physician (for those who acquire insurance)

Help the patient identify a new Primary Care Physician (does not necessarily need to be completed in clinic)You can ask your Pt. whether s/he wants assistance in finding a new PCP and inquire about location, etc. preferences. IMA, IFH, and Boriken are some nearby clinics - depends on patient plan and patient preferences

If seeking a PCP outside of IFH/IMA, etc.

Google “Insurance company name providers”Identify possible PCPs accepting the specific plan with consideration for Pt’s location preferencesCall these physicians, officesConfirm PCP accepts planAsk if desired PCP is currently accepting new patients under this planProvide patient with a list of possible physicians, encourage Pt. to contact his/her insurance company to verify that the plan covers this physician
