Senior Clinician

Place an order in Epic (please place specific order from list above).Before clinic ends, communicate the necessity and urgency of the referral to the TS.

Teaching Senior

Verify there is an active order in Epic.Add the referral to the ACT Referrals Spreadsheet, "Referrals Request" tab and include the patient's availability for scheduling (day/time).

Access to Care Team

Make sure the patient has charity care. If not, please fill out the REAP Referral form.Get patient's availability and add to their ACT Problem List in Epic.

Wait time

The wait time will vary depending on how long it takes a patient to obtain charity care. After the patient obtains charity care, it takes about 3-4 weeks for the PFT to be scheduled.
