For polysomnography, the Ambulatory Referrals Manager should email Dr. Shah.

Senior Clinician

Place an order in Epic.Before clinic ends, communicate the necessity and urgency of the referral to the TS.

Teaching Senior

Verify there is an active order in Epic.Add the referral to the ACT Referrals Spreadsheet, "Referrals Request" tab and include the patient's availability for scheduling (day/time).

Access to Care Team

Make sure the patient has charity care. If not, please fill out the REAP Referral form.Get patient's availability and add to their ACT Problem List in Epic.

Cost

Cost to EHHOP Patient: Once your patient submits documents confirming their income, Financial Services will grant you patient Charity Care and place them on a sliding scale. Most EHHOP patients are placed on Level 1. For patients with Level 1, the Pulmonology appointment will cost $10. To find out the costs for patients who are not Level 1, please contact the Ambulatory Referrals Manager, Marc Casale.

Wait time

How long it takes patients to complete their appointment will depend on how soon the patients can submit all their documents confirming their income to Financial Services (on average this takes 20 days). Once all documents are submitted, it takes about 3 weeks after a patient has received charity care for a pulmonology appointment to be scheduled.
