Due to the backlog of patients from the COVID-19 pandemic, the pulmonology department at Mount Sinai is Hospital is not able to offer sleep studies to EHHOP patients at this time (2/11/21). The EHHOP team is currently working to establish a new partnership. Please email Chief TS for updates. See below for previous workflow.
To request a sleep study, senior clinicians should email the following information to the Chief TS (Brittany Glassberg):
- Mallampati score
- Epley sleepiness scale
- STOP-BANG score
- BMI
- Symptoms of OSA
This referral requires charity care.
Senior Clinician
- Place an order in Epic.
- Before clinic ends, communicate the necessity and urgency of the referral to the TS.
Teaching Senior
- Verify there is an active order in Epic.
- Add the referral to the ACT Referrals Spreadsheet, "Referrals Request" tab and include the patient's availability for scheduling (day/time).
Access to Care Team
- Make sure the patient has charity care. If not, please fill out the REAP Referral form.
- Get patient's availability and add to their ACT Problem List in Epic.
