This referral does not require charity care

For difficulty with NEAR vision (presbyopia):

Use main clinic reading glasses program

For difficulty with DISTANCE vision

  • Please set the patient up for an appointment with EHHOP Ophthalmology.

  • EHHOPhtho clinic managers will then either:

    • Order the glasses from Grand Central Optical
    • Refer the patient to LensCrafters OneSight program

For lost glasses with recent PRIOR prescription (OneSight Program):

  • The Senior Clinician, Teaching Senior or Attending Physician can fill out the referral letter: LenscraftersReferralLetter

  • Give the patient the completed referral letter and a copy of his/her glasses prescription. (Check with EHHOPhtho CMs Yoni Goldstein and Monica Kim if you cannot find the prescription).

  • Patients can walk-in with these documents to the following locations at the following times. They can choose frames from the store's OneSight stock, and 2 weeks later they will need to return to the store to pick up their glasses.
