Eye Glasses

Please submit the ancillary clinic referral form and write that a glasses prescription is needed

For difficulty with NEAR vision (presbyopia):

Use the main clinic reading glasses program.

For difficulty with DISTANCE vision

  • Please set the patient up for an appointment with EHHOP Ophthalmology.

  • EHHOPhtho will triage the patient:

    • Determine decreased visual acuity is due to refractive errors or other causes
    • Refer the patient to LensCrafters OneSight program if glasses are needed

For free optometry eye exam/new frames once prescription is obtained (OneSight Program):

  • The Senior Clinician or Teaching Senior should add a referral request to the ACT Referrals Spreadsheet Referral Request tab.

  • ACT Referrals Chair will schedule the patient for an appointment and send in their verification letter to LensCrafters. The ACT Referrals Chair will then let the patient's ACT case manager/chronic care senior know about the patient's appointment time. The patient can choose frames from the store's OneSight stock, and 2 weeks later they will need to return to the store to pick up their glasses.

  • If the patient thinks they will have difficulty getting to LensCrafters on time for their appointment, the patient's case manager or chronic care senior can order the patient an Uber and apply for reimbursement from EHHOP.
