ALL patients should be asked the following two questions to screen for food insecurity:
If the response to either question is "yes",
- you MUST inform the ACT Member in clinic
so that ACT can further evaluate and counsel the patient. The ACT member will provide the patient with the resources to access food. Make sure to inform the Head TS during sign out of any positive Food Insecurity screens.
The NY Common Pantry is open on Saturdays for intake screens and only requires a photo ID to register. The patient may be able to leave with fresh and nutritious groceries the same day. If the patient does not have a photo ID, we can assist them with obtaining a NYC ID.
