Food Insecurity Screening & Referral

ALL patients should be asked the following two questions to screen for food insecurity:

0-0.9 Negative good
1-2 Positive bad
1. Within the past 12 mo, we worried whether our food would run out before we got money to buy more.
2. Within the past 12 mo, the food we bought just didn’t last and we didn’t have money to get more.

If the response to either question is "yes",

so that ACT can further evaluate and counsel the patient. The ACT member will provide the patient with resources to access food. Make sure to inform the Head TS during sign out of any positive Food Insecurity screens.

The NY Common Pantry is open on Saturdays for intake screens and only requires a photo ID to register. The patient may be able to leave with fresh and nutritious groceries the same day. If the patient does not have a photo ID, we can assist them with obtaining a NYC ID.

Here is the form for a PAF emergency food delivery if required.
