How do referrals to Mental Health, Optho, and Women's Health work?

5 Step Overview:

  1. TS sends out referral to referrals@mssm.edu By Sunday 5PM. The TS does not need to put a referral into Epic.
  2. Specialty TS “on call”* for that week determines urgency of appointment by Monday 5PM.
  3. Specialty CM “on call”# for that week schedules patient by Tuesday 5PM based on TS recommendations and informs patient of appointment.
  4. Once referral is scheduled, CM goes into Epic problem list, and moves referral from “Outstanding referrals” to “Referrals in progress,” including the date the problem list is updated by the CM as well as the date of the appt. Additional instructions are below.
  5. Once patient has attended the referral, close the loop by moving the referral from “Referrals in progress” to “Completed referrals.”

* TS on call = Chief TS for GYN and rotating as per AMION EHHOP for Ophtho and MH

# CM on call = rotating duty as per AMION EHHOP

$ Loop closer = In clinic TS for GYN and Ophtho, In clinic CM for MH


Updating the problem list

Step 1: Go into the patient’s chart in Epic. Once you get into the patient’s chart in Epic, go into the problem list, located on the left side of the screen.Screen Shot 2013-05-04 at 6.36.35 PM

Step 2: Go into the ACT/Referrals tab in the problem list.

Step 3: This is the page you’ll see. The sections you will update already exist. Double click on the section itself to edit it.

Step 4: The page should allow you to edit it now.

Step 5: Hit accept. You will see a warning pop up. Click OK.

Then, you’re good to go!
