Senior Clinician

Place an order for the referral in Epic.Before clinic ends, communicate the necessity and urgency of the referral to the TS.Inform ACT about the referral and send the patient to ACT.

Teaching Senior

Communicate the referral to ACT.

Access to Care Team

Make sure the patient has charity care.If not, follow the process to obtain charity care for the patient.Once the patient has charity care, obtain the patient's availability, and contact the Ambulatory Referrals Manager to make the appointment.Then, call the patient to see if he/she can attend the appointment.

Cost

The cost of an appointment for patients with charity care level 1 is $10. Appointments will be more expensive for patients with a higher charity care level.

Appointment costs by charity care level are as follows:

CCL1 - $10
CCL2 - $18
CCL3 - $36
CCL4 - $90
CCL5 - $108
CCL6 - $135
CCL7 - $180

Wait time

Once the patient has charity care, it takes about 10 weeks for an ophthalmology appointment to be scheduled, but the wait time is variable by time of year.
