This referral requires charity care.

Senior Clinician

Place an order for the referral in Epic.Before clinic ends, communicate the necessity and urgency of the referral to the TS.Inform ACT about the referral and send the patient to ACT.

Teaching Senior

Include the necessity and urgency of the referral in the weekly referrals@mssm.edu email.

Access to Care Team

Make sure the patient has charity care. If not, follow the process to obtain charity care for the patient.Once the patient has charity care, contact Retina to make the appointment.Call the patient to see if he/she can attend the appointment.Email the Emerging Referrals Manager to confirm.

Visit the Financial Services page when you arrive at your patient's FS appointment for further directions.

Once your patient has charity care, please contact the Ambulatory Referrals Manager. The Ambulatory Referrals Manager will schedule your patient for their specialty referral appointment.

Cost

The cost of an appointment for patients with charity care level 1 is $15. Appointments will be more expensive for patients with a higher charity care level. For these prices, contact the Emerging Referrals Manager.

Appointment costs by charity care level are as follows:

CCL1 - $15
CCL2 - $18
CCL3 - $36
CCL4 - $90
CCL5 - $108
CCL6 - $135
CCL7 - $180

Wait time

It takes about 6 weeks after a patient has received charity care for a retina appointment to be scheduled. Urgent referrals can be expedited. Contact George Mellgard if the referral is urgent and needs to be expedited.
