PAF Spending Guidelines Defitions
Typical Expenditures:
Anything less than $300 dollars that covers an urgent need directly impacting patient health. Examples include:
Food delivery for patients facing urgent food insecurity who cannot easily access community pantries.
Transportation to and from medical, mental health and social work appointments via public transportation.
Costs for expensive essential prescribed medications that are awaiting approval from a Patient Drug Assistance Program (PDAP).
Specialty procedures for which, even at reduced prices granted by the hospital for charity care procedures, are still unaffordable for the patient.
Durable medical equipment such as walkers, canes, diabetic shoes, splints, hearing aids, blood pressure cuff, etc.
Other emergency expenditures that may directly impact health (i.e. housing or heating bills, prepaid phone minutes, microwave, mattress, winter coat etc).
Atypical Expenditure:
Anything that costs more than $300 or that does not directly impact patient health. For more information about consideration of large PAF Expenditures, please see the extended PAF Protocols
PAF Purchase Requests:
1) Submit this form for purchase requests from Amazon.
2) Submit this form for an emergency grocery delivery.
3) If you had to cover the money for a PAF purchase out-of-pocket, fill out this form and request a Venmo reimbursement from Dr. Meah (@Yasmin-Meah).
