Select “Hospital Based Practices Referral” Mount Sinai St. Luke’s (4313) Input reason for consult in “comments” section Referring provider contact number: EHHOP main number Referral type: most often will be "consult, diagnostic procedure, treat" Once you've associated the order with a diagnosis and pended it, you'll need to send a staff message to Jessica Tran, Dr. Meah, and David Skovran telling them the date of the office visit you pended the order to and asking them to sign.

Cost

The cost of an appointment for patients with charity care level 1 is $10. Appointments will be more expensive for patients with a higher charity care level.

Appointment costs by charity care level are as follows:

  • CCL1 - $10
  • CCL2 - $18
  • CCL3 - $36
  • CCL4 - $90
  • CCL5 - $108
  • CCL6 - $135
  • CCL7 - $180

Wait time

It takes about 1 month for an ophthalmology appointment to be scheduled.
