Cost to EHHOP Patient: None; EHHOP covers all radiology visits

Senior Clinician

Place an order in Epic.Before clinic ends, communicate the necessity and urgency of the referral to the TS.

Teaching Senior

Verify there is an active order in Epic.Add the referral to the ACT Referrals Spreadsheet, "Referrals Request" tab and include the patient's availability for scheduling (day/time).

Access to Care Team

Make sure that the referral has been ordered and signed in EPIC.Get patient's availability and add to their ACT Problem List in Epic.

Pre-COVID: No appointments are required for x-rays, nor is charity care. Instruct patient that they can walk in at any time, Wednesdays (8:30AM - 3:00PM) to 17 E 102nd St, 6th floor for their appointment.The Radiology Referrals Manager will give the completed patient X-ray Referral form with his/her name and MRN filled out to the Radiology department.The Radiology Referrals Manager will follow up at the end of the week to see if the appointment is made, first by checking in EPIC, and then by calling the patient.Inform patient that they can call EHHOP at (646) 942-6519 with questions or to request someone to attend the appointment with him/her.

Wait time

Appointments are walk-in. Patients can receive their X-Rays whenever they go to 6th floor CAM on Wednesdays 8:30-3:00 (COVID-19 schedule, call to confirm).
