Checking Out Patients

  1. Red Light the Patient in EPIC: Go to Schedule --> Highlight the Patient --> Left click in the Left most Column --> Select the Red Light
  2. Print the Patients After Visit Summary if instructed by the clinical team: Select the patient off of the Schedule Go to “Print Follow Up” Give this to the patient
  3. Give patient any voucher(s), scripts, or Patient Instructions for Referrals as instructed by the clinical team.
  4. If there are forms (ie. EKGs) that need to be scanned into EPIC, fill out Scan Request Form. Forms and To-Be-Scanned tray is in Attendings’ room on the shelf on the left wall. Paper-clip request form and documents to be scanned and leave in tray.
  5. Make the patient’s next visit: Make the appointment in CERNER after checking EPIC for any upcoming appointments.
  6. Jot down the date and time on one of our EHHOP cards and give it to the patient; IF YOU GIVE A PATIENT AN APPOINTMENT CARD, MAKE SURE YOU WRITE DOWN WHAT THE APPOINTMENT IS FOR (CLINIC, LABS, SW, ETC), AND CONFIRM THAT EACH APPOINTMENT IS ON CERNER BEFORE YOU SEND THE PATIENT HOME.
  7. If the patient needs to be fasting, make them a 9am appointment. Late appointments for fasting patients → cranky patients
  8. If there are special instructions that the patient must be reminded of when the CM calls them before their next appointment (ex: pt needs to be fasting before next labs appt, patient must be seen by a chronic care senior), be sure to add a visible note to their Cerner appointment.
  9. Check out in Cerner: Go to the patient’s appointment slot. Right Click --> Actions --> Check Out All patients must be checked out at the end of clinic day.
  10. If an appointment isn't checked out, it creates a problem in EPIC and the chart won't be able to be closed.
  11. Keep a list of all patient issues that need to be addressed after clinic day—EPIC/Cerner name corrections, patients coming in to drop off specimen samples, other reminders, etc. -Send this list to the Chief CM at the end of the day.
