Cost: Community Healthcare Network (CHN) - Harlem Health Center will grant your patient a sliding scale immediately the day that they submit their income/support documentation. Most EHHOP patients are placed on Fee Level A1 (Fee is 10% of full price charged). For patients with Level 1, the initial dentistry visit will cost $25 (includes mandatory xray and cleaning). Documents Required: 1) ID (passport from another country will suffice) 2) proof of address (any piece of mail with name, letter from whom they are renting from or living with) 3) proof of income (either pay stubs, letter of support, or printed bank statement).
