General notes:
- The EHHOP TS sign-out text area is ONLY located in the Visit Navigator for each Patient Encounter (ie: Office Visit)
- As you know, you can access the Visit Navigator by clicking an open chart through the Schedule the day of clinic when the chart is still open (only the attending closes this)
- To access Visit Navigator after clinic day or sometimes, during the clinic day after the attending has closed the chart, you must create an Addendum to the Encounter. Epic will prompt you automatically about whether or not you want to create an Addendum if you try to open it from the Schedule after the chart has already been closed. OR you can click on the big Epic button at top left corner –> create addendum –> find the appropriate office visit.
- Once you locate visit Navigator, click on the last section: "EHHOP TS Sign-Out"
- Enter the
.TSPLANOFCARE
smart text to bring up the template - Put the prelim sign-out in the chart as described above and when you complete the sign-out, just go back into the encounter if the chart is still open or create an addendum.
1) Please note that this EHHOP TS Sign-out then gets filed into the patient’s chart and can be found in its own unique category, titled “Plan of Care“.
- Unfortunately, Epic has default category names, and we could not change this to say “Sign-out,” which would have been ideal. If you were not in Schedule, to find the last plan of care go to Chart Review → Notes → Look for the last Plan of Care (which will = last TS Sign-out)
- Note: Plan of care will only be visible under Notes if the Attending closed the chart. If the Attending did not close the chart, the Sign-out will only show up under Schedule and in the Encounter note (see "Accessing prior Epic Sign Outs" tab for instructions on how to access)
2) Now if we go back to the Schedule view, note that clicking on a Patient will bring up the last EHHOP TS Sign-out.
3) Finally, the Sign-out also populates into the Office Visit – Encounter view from Chart Review → Encounters. It comes up after the brief Attending Progress Note. This is possible to view even if the Attending has not closed the chart.
