If a patient does not meet the three criteria to be an EHHOP patient OR if we are at capacity, TS/CMs for the week should kindly inform them that we will unfortunately not be able to provide them with medical care. Let them know that we can give them the number of another free clinic. You can also offer for them to speak with a social worker if there are additional resources that they are looking for.
If they are interested in speaking with a SW, then please email their contact information to the SW for the week explaining the reason for the call.
