Senior Clinician

Before clinic ends, communicate the necessity and urgency of referral to TS.Endoscopy Center of New York referral form must be filled out by clinic team or TS, found below. Form must also be signed (send to Brittany Glassberg at Brittany.glassberg@icahn.mssm.edu to get Dr. Meah's signature).Be sure to completely fill out the referral form, including height/weight/phone/medical history/current medications and have the attending sign the form.Upload form to ECNY Referral folder in the ACT Google Drive (ACT > Referrals > ECNY Forms)Add referral to the ACT Referrals Spreadsheet Referrals Request tab.Prep is covered by EHHOP clinic and is given to patient in clinic once the appointment has been scheduled (prep stored in IMA lockers).

SYMPTOMATIC COLOONOSCOPIES

In clinic, get the patient's availability, and have clinic team or TS fill out the ECNY referral form, which can be found below. Save to the "ECNY Forms" folder in the ACT drive (EHHOP Drive > ACT > Referrals > ECNY Forms).Teaching senior needs to add the referral to the ACT Referrals Spreadsheet Referrals Request tab.The Accessory Referrals Manager, Abrisham Eskandari, will email the Endoscopy Center of New York's Patient Navigator, Yenicey Roman, at yroman@ecny.com to schedule an appointment.Patient should come to clinic on the weekend before their procedure to pick up the colonoscopy prep and a copy of the colonoscopy instructions. If they can't come into clinic, their case manager is responsible for coordinating prep pick up with them. Cost

Symptomatic colonoscopies are currently free of charge.

Symptomatic colonoscopies may require charity care if the procedure cannot be done at the Endoscopy Center of New York. If this is the case, REAP will grant your patient Charity Care and place them on sliding scale financial assistance once your patient submits documents confirming their income. Most EHHOP patients are placed on Level 1. For more information, please contact the Accessory Referrals Manager.

Wait Time

There is a 1-2 month wait time for symptomatic colonoscopies.
