Instructions for referring patients to other clinics:
If a patient is ineligible for EHHOP OR if we have determined that we are at capacity, we will refer patients out to other clinics in the area. TS/CMs for the week should call any potential new patients looking for care and refer them to one of the clinics below or in the linked google doc. If patients are in need of other resources, you can email the SW for the week with their information (name, phone number, language, location, age), however please make sure to use your discretion to protect our SW's time.

Services: Primary care program in New York specifically designed for the health needs of adolescents. The Adolescent Health Center provides confidential comprehensive medical, mental health, family planning, and health education services to young people between the ages of 10-26.

Services: EHCHS is a not-for-profit community-based organization established in 1965 to provide crucially needed services to residents of East Harlem. Today, EHCHS is a bilingual/bicultural organization providing a broad range of services to a largely Latino clientele. Full health and dental with pharmacy among other social programs.

Services: Mission is to provide East Harlem/El Barrio with high quality, comprehensive, accessible and culturally sensitive preventive and primary care services, and to coordinate the other levels of health care that are necessary. We include women’s health, pediatrics, adult medicine, podiatry, HIV medicine and case management, and social services.

Services: Over the years, programs have expanded in response to community needs. They now include the Advocacy Office and Emergency Food Pantry, the Family Support (Preventive Services) Program, the Early Childhood Program, the Community Life Program and The Sharing Place thrift store. The Home Health Agency program offers nursing care to those with diabetes or asthma, new mothers and babies, people living with HIV or AIDS, and the homebound elderly.
