Review the med list
Pull up the Medications section of the Visit Navigator
- Open with, "What medications are you taking?"
Let them think and speak for awhile before you:
- Ask them, "Did you bring your medications with you? Can you take them out so we can look at them together?"
Now instruct and ask them close-ended questions:
You should be taking this medication (hold up the bottle) called Lisinopril for your blood pressure and to protect your kidneys. Take 40 mg (that's two tablets) once a day. Have you been taking this? When was the last time you took this medication?
Okay, from now on, (repeat instructions).
Clean up the med list
Discontinue
- Duplicate meds
- Meds patient is no longer taking (and no longer needs to take).
