A Nutrition Corps member will see your pt longitudinally. In our visits, we implement motivational interviewing, behavioral health coaching, and lifestyle modification education. We see pts that are food insecure as well, so please note in form if they screen positive or have a significant history of food insecurity.

Cost

There is no cost to the patient to speak with Nutrition Corps.

Wait time

Nutrition Corps will try to see patients as soon as Nutrition Corps member is available. If you need assistance, please reach out to the Nutrition Corps Chair (see EHHOP Roster).
