Most Rheumatology referrals are CURBSIDED. SEE COMPLETE INSTRUCTIONS IN THE TABS BELOW. Full visits require charity are.

Senior Clinician

Place an order in Epic (Please place an Econsult order to start unless specifically decided otherwise, the Rheum consult fellow will escalate to an in person visit if necessary)Before clinic ends, communicate the necessity and urgency of the referral to the TS (very urgent or difficult consultations could be escalated to Dr. Kerr, please do this judiciously and generally Dr. Meah will make this distinction)

Teaching Senior

Verify there is an active order in Epic (Should probably be an Econsult unless specifially discussed)Add the referral to the ACT Referrals Spreadsheet, "Referrals Request" tab and include the patient's availability for scheduling (day/time) - IF AN IN PERSON CONSULTATION.

Access to Care Team

Dr. Leslie Kerr may assists with these referrals, so patients will not need EMC/CC if that is the case. Alternatively, no visit needs to be scheduled for an Econsult.If Dr. Kerr is not available, then Ambulatory Referrals Manager will schedule at the resident clinic and EMC will be required.Make sure the patient has charity care. If not, please fill out the REAP Referral form.Get patient's availability and add to their ACT Problem List in Epic.
