Eligibility

Patients who live outside of East Harlem (including shelters)Patients who have referral appointments outside of East HarlemACT Case Managers and Social Workers who are accompanying patients to referrals away from Sinai (i.e. IFH, St. Lukes, Mount Sinai West)

Protocols

ACT Chairs identify patients in pre-screen that may be eligibleIf positive for need ACT case managers and CM’s give the patients one 2 way metro card for each referrals they have and/or one 2 way metrocard if they live outside of East Harlem.The ACT case managers will include any metrocards they gave out in their post clinic notes with the amount given out, date, and purposeMETROCARDS will need to be purchased in advance in bulk through a direct fund transfer by contacting Jacklyn Njegric, Brendan Bullen and the EHHOP treasurer.All Metrocards will be stored in a locked cabinet in EHHOP and Case Managers will need to log when METROCARDS are dispensed to patients in the post-clinic emailEach purchase must be logged into a GOOGLE form (see bottom of page)Should METROCARDS not be available or a cab is deemed the safer mode of transport due to patient’s debility, a student, social worker or faculty member may purchase the METROCARD or CAB ride and submit the receipt for reimbursement in same way as mentioned previously

Metrocards are kept in IMA locker number 7228 and the code is 21-35-05.The CM on duty can help if there are any problems.
