-If NO, patient is eligible for EHHOP services. Next Steps:
a) Ask if the patient has ever received healthcare elsewhere. If yes, please have patient fill out a medical release form and follow the protocol to receive the medical records. Medical records.
b) Go over the Patient Information Sheet in the EHHOP Patient Folder (please ask a CM for a folder): English Spanish
c) Please send patient to front desk clinic managers to schedule new patient appointment.
-If YES, patient is not eligible for EHHOP services. Proceed to the following questionnaire to determine if patient requires a bridge to care visit: Ineligible Patients. If patient answers NO to all questions, please refer patient to REAP or IFH to apply for insurance. If patient answers YES to any of the questions, please give the questionnaire to the head Teaching Senior who will determine whether the patient needs an urgent visit or bridge-to-care visit.
